Debit Card and ATM Application

To reissue or request a new Debit/ATM Card or request a new PIN, please fill out our Debit and ATM Card Application and submit it to your local branch or mail it to: 2030 Sea Level Drive, Ste 200 Ketchikan, AK 99901.

If you are an existing customer with an existing debit card and you would like your card or PIN reissued due to damage or you have lost your Debit card and would like a new Debit card and PIN you can sign on to your personal PowerNET and submit the request through your mailbox. This is NOT an option for a new customer or a customer that has not had an ATM/Debit card in the past 3 months.
